Requirements
- RN, LCSW, or CHW credential; care management experience
- ECM or CCM program experience strongly preferred
- Multilingual skills valued — Vietnamese, Tagalog, Spanish, or Punjabi for Fremont/Newark populations
- eClinicalWorks or similar EHR documentation skills
- Community-based outreach and field work capacity; driver's license required
