About the Role
CHW at The Clinic connecting LA's under-resourced communities to health and social services across 7 sites. Conduct outreach, SDOH screenings, and patient navigation for Medi-Cal members and uninsured LA residents seeking access to preventive and primary care.
Requirements
- CA CHW certification (SB 803) preferred
- Bilingual Spanish/English required
- LA County social services and Medi-Cal knowledge
- Community outreach and motivational interviewing
- Valid California driver's license preferred
