Outreach and navigation for UHC's migrant and seasonal agricultural worker patient population in Fresno and Tulare counties — connecting farmworkers to primary care, enrollment, and social services.
Requirements
- CHW certification or 2+ years outreach with agricultural communities
- Spanish fluency required; Mixtec or other indigenous language a strong plus
- Valid CA driver's license — field travel across agricultural areas required
- Personal connection to farmworker communities highly valued
