Coordinate Enhanced Care Management for UHC's high-need Medi-Cal patients — care plans, social service navigation, and coordination across medical, behavioral, and community resources in the Central Valley.
Requirements
- RN, SW, or MA with 2+ years care coordination experience
- Spanish fluency required
- OCHIN Epic preferred
- Agricultural community knowledge a plus
