Provide Enhanced Care Management outreach and support to high-risk patients in Fresno County agricultural communities. Combine CHW engagement skills with MA clinical tasks. Home visits and community outreach required.
Requirements
- CHW certification or Medical Assistant certification
- Bilingual Spanish/English required (agricultural community)
- Experience with community outreach and patient engagement
- Valid California driver's license and reliable transportation
