Coordinate care for Venice Family Clinic's most complex patients — homeless individuals, HIV+ patients, and those with co-occurring BH/SUD — developing care plans and linking to housing and social services.
Requirements
- RN, SW, or 3+ years complex care coordination experience
- Experience with homeless, HIV, or BH populations required
- Spanish preferred
- OCHIN Epic preferred
