ECM/CCM care manager at Westside Family Health Center serving 14,000+ LA Westside patients. Manage complex Medi-Cal members including unhoused individuals, chronically ill, and high-utilizers. Santa Monica and surrounding Westside communities.
Requirements
- RN, LCSW, or CHW credential; ECM or CCM experience strongly preferred
- Experience with housing-unstable or unhoused patient populations
- Bilingual Spanish a plus for Latino families on the Westside
- eClinicalWorks documentation skills
- Community-based field work capacity; driver's license required
