CHW at Yehowa Medical Services connecting South Bay and South LA communities to health and social services across 10 sites. Outreach, SDOH screenings, and patient navigation for Medi-Cal members and uninsured residents in Torrance, Carson, Compton, and surrounding communities.
Requirements
- CA CHW certification (SB 803) preferred
- Bilingual Spanish/English required
- LA County social services and Medi-Cal navigation
- Community outreach experience in South Bay/South LA
- Motivational interviewing and trauma-informed care
