Peninsula Healthcare Connection (PHC) is a nonprofit FQHC Lookalike founded in 2006 and officially designated in 2019, serving homeless and low-income populations in Santa Clara County from its clinic at the Opportunity Center in Palo Alto. PHC provides primary care, psychiatry, dermatology, case management, and community-based outreach through its Backpack Medicine program, in partnership with Stanford Health Care and Palo Alto Medical Foundation.
