Peninsula Healthcare Connection (PHC) was founded in 2005 as a collaboration between Palo Alto Medical Foundation, Stanford University School of Medicine, and community partners. CA-licensed clinic since 2011; officially designated FQHC-LAL (Look-Alike) in 2019. Sole clinic at the Opportunity Center in Palo Alto serves Santa Clara and South San Mateo counties. Unique 'Backpack Medicine' street outreach program brings Stanford/PAMF physicians to homeless patients in the field. New Directions program offers community-based behavioral health and case management referred from county/Medi-Cal partners.
