School Health Clinics of Santa Clara County has operated since 1986. Independent 501(c)3 since July 2003 (split from Health Trust to retain federal funding/FQHC status). Runs five FQHC clinics embedded on public school campuses in San José and Gilroy, serving 4,500+ low-income patients annually. Each clinic staffed with NP or PA + 2 bilingual MAs + supervising physicians under Medical Director Dr. Claude Rogé (pediatric cardiologist). CEO Stephanie (former Kaiser critical care nurse → SHC Family NP since 2015). At least Overfelt Neighborhood Health Clinic appears in NCQA Report Cards directory. County of Santa Clara is funding EHR conversion from NextGen to Epic for interoperability.
