CMS adds heart failure and addiction tracks to ACCESS model
Issue
CMS said on September 15, 2026 that its ACCESS chronic-care model will add heart failure, COPD, substance use disorder and tobacco cessation tracks. The new tracks start in spring 2027, and 160 organizations now take part.
Primary care teams can refer Original Medicare patients to ACCESS participants listed at Medicare.gov/ACCESS.
