Requirements
- BA/BS in social work, public health, or related field; or 3+ years FQHC care coordination
- Spanish fluency required
- ECM or CCM program experience strongly preferred
- OCHIN Epic proficiency preferred
- Valid CA driver's license for community-based care visits
