About the Role
Supervise nursing staff and coordinate interdisciplinary care for AltaMed's Program of All-Inclusive Care for the Elderly (PACE), managing complex geriatric patients in a capitated, community-based model.
Requirements
- California RN license required; BSN preferred; geriatric or PACE experience strongly preferred
- 2+ years supervisory nursing experience; case management skills required
- Experience with Medicare/Medi-Cal dual eligibles and PACE enrollment
- BLS/CPR required; knowledge of CMS PACE regulations
