About the Role
Provide primary and specialty geriatric care to AltaMed's PACE and senior patient populations in Orange County, focusing on chronic disease management, functional assessment, and geriatric syndromes.
Requirements
- MSN-NP with California NP license and DEA registration; Geriatrics certification preferred
- 2+ years geriatric or PACE NP experience in FQHC or senior care setting
- NHSC loan repayment eligible — AltaMed qualifies as HPSA site
- Familiarity with FQHC PPS billing and Medicare dual eligible patients
