About the Role
Provide psychosocial assessments, counseling, and care coordination for AltaMed's senior population in PACE and comprehensive care programs, supporting functional independence and community living.
Requirements
- LCSW with current California license; geriatric social work experience preferred
- PACE, ADHC, or senior care program experience a strong plus
- Familiarity with Medi-Cal/Medicare benefit navigation and community resources
- Spanish language skills preferred for OC senior population
