About the Role
Deliver hybrid inpatient and community-based palliative care to AltaMed's seriously ill patient population, focusing on goals-of-care conversations, symptom management, and care coordination with PACE and CCM teams.
Requirements
- NP with California license and national board certification (FNP, AGNP, or ACHPN)
- Palliative care certification or specialized training strongly preferred
- NHSC loan repayment eligible; bilingual Spanish/English a strong plus
- Experience with POLST, advance care planning, and serious illness communication
