About the Role
Provide Ryan White-funded medical case management for AltaMed's HIV-positive patient population, coordinating ADAP enrollment, adherence support, social services linkage, and care plan development for under-resourced LA communities.
Requirements
- BA/BS in social work, health education, or related field; CHW or case manager certification preferred
- HIV case management experience, Ryan White services knowledge, and ADAP familiarity required
- Bilingual Spanish/English preferred; experience with undocumented patient navigation a plus
- Familiarity with harm reduction, PrEP/PEP, and LGBTQ+ affirming care practices
