About the Role
Deliver physical therapy services to AltaMed PACE participants, conducting functional assessments, developing therapeutic exercise programs, and reducing fall risk for frail elderly members enrolled in the PACE day center.
Requirements
- PT with California license required; DPT preferred; geriatric or PACE experience strongly preferred
- Experience with functional mobility assessments, fall prevention protocols, and IDT care planning
- Familiarity with Medicare/Medi-Cal PACE therapy documentation and coverage rules
- BLS/CPR required; Spanish language proficiency a plus
