About the Role
Provide palliative care consultations and serious illness management for AltaMed's complex patient population, conducting goals-of-care conversations, pain and symptom management, and advance care planning across clinic and PACE settings.
Requirements
- PA-C with California license; palliative care or hospice experience preferred
- Training in serious illness communication, POLST completion, and advance care planning
- NHSC loan repayment eligible; bilingual Spanish/English a strong asset
- Comfortable in interdisciplinary team environment across PACE and outpatient settings
