About the Role
Provide comprehensive nutritional assessments, therapeutic diet counseling, and meal planning support for AltaMed PACE participants, managing malnutrition, diabetes, renal disease, and dysphagia in a frail elderly population.
Requirements
- RD or RDN credential required; gerontology or PACE nutrition experience preferred
- Experience with MNA malnutrition screening, therapeutic diets, and tube feeding management
- PACE IDT participation and care plan documentation experience preferred
- Spanish language skills a plus; BLS/CPR required
