CHW supporting ECM at Arroyo Vista's Northeast LA sites. Field-based work in Highland Park, Eagle Rock, Boyle Heights — connecting high-need Medi-Cal patients to primary care, housing, food, and social services.
Requirements
- CHW certification or 2+ years community health outreach experience
- Spanish fluency required
- Knowledge of Northeast LA community resources and Medi-Cal enrollment
- Valid CA driver's license preferred
- Lived experience in the Northeast LA community highly valued
