Requirements
- BA/BS in social work, health education, or related field; or 3+ years ECM/care coordination
- Spanish or Tagalog fluency preferred
- Valid CA driver's license for community-based ECM visits
- OCHIN Epic documentation experience preferred
- Culturally responsive care for Filipino, Afghan, and Latino patients required
