CHW at BACH connecting Fremont and Newark's diverse immigrant communities to primary care, Medi-Cal, WIC, and social services. Multilingual outreach workers especially needed — Tagalog, Dari/Farsi, Spanish.
Requirements
- CHW certification or 2+ years community health outreach experience
- Tagalog, Dari/Farsi, or Spanish fluency highly preferred
- Valid CA driver's license
- Knowledge of East Bay social services, Medi-Cal, and immigrant resources
- Lived experience in Fremont/Newark/Union City community a plus
