Requirements
- RN, LCSW, or bachelor's in social work/nursing with 2+ years care management experience
- ECM or complex care management experience preferred
- CalAIM and Medi-Cal program knowledge
- Community-based, field-facing care management skills
- Social determinants of health documentation and navigation
