Requirements
- BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
- Spanish fluency required
- OCHIN Epic documentation experience preferred
- Valid CA driver's license for field-based ECM visits
- Knowledge of San Joaquin and Stanislaus County social services
