Requirements
- BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
- Spanish fluency required
- OCHIN Epic documentation experience preferred
- Valid CA driver's license for community-based ECM visits
- Knowledge of Yolo County social services, housing resources, and Medi-Cal
