CHW at Equity Health connecting San Francisco's under-resourced communities to health and social services across 6 sites. Focus on SDOH navigation, outreach to unhoused and immigrant communities, and linkage to housing, food, and mental health resources.
Requirements
- CA CHW certification preferred (SB 803)
- Experience with unhoused, LGBTQ+, or immigrant communities in SF
- Knowledge of SF DPH and community resource landscape
- Motivational interviewing and trauma-informed outreach
- Second language preferred (Spanish, Cantonese, Tagalog)
