About the Role
Provide skilled nursing case management for FHCSD's PACE program, coordinating comprehensive care for frail elderly participants and collaborating with the interdisciplinary care team.
Requirements
- California RN license required; BSN preferred; geriatric or PACE experience preferred
- Case management certification (CCM) or equivalent experience a plus
- Experience with Medicare/Medi-Cal dual eligible populations
- BLS/CPR required; OCHIN Epic proficiency preferred
