About the Role
Provide social work services for FHCSD's PACE frail elderly population, conducting comprehensive psychosocial assessments, developing care plans with the IDT, and connecting participants to community resources and housing supports.
Requirements
- MSW with California LCSW or ACSW registration required; geriatric social work experience preferred
- PACE IDT experience, care plan development, and CalAIM social needs screening familiarity
- Bilingual Spanish/English preferred for FHCSD's diverse San Diego participant population
- Knowledge of Medicare/Medi-Cal dual-eligible PACE benefit structure and disenrollment processes
