About the Role
Provide comprehensive social work services for AltaMed PACE frail elderly participants, coordinating community resources, conducting psychosocial assessments, and participating in interdisciplinary team care planning.
Requirements
- MSW with California LCSW or ACSW registration; geriatric experience preferred
- Bilingual Spanish/English required for East LA and Montebello PACE participant population
- Experience with Medicare/Medi-Cal PACE dual-eligible benefit structure
- PACE IDT participation, care planning, and social needs documentation experience
