About the Role
Provide embedded social work services within FHCSD's primary care teams, conducting social needs screenings, connecting patients to community resources, and supporting complex care management for Medi-Cal members with unmet social determinants of health.
Requirements
- MSW with California LCSW or ACSW registration; primary care or FQHC social work experience preferred
- Familiarity with CalAIM SDOH screening tools, ECM referral pathways, and San Diego community resources
- Bilingual Spanish/English preferred for FHCSD's predominantly Spanish-speaking patient population
- Experience with trauma-informed care, motivational interviewing, and community resource navigation
