Medical Director for Mission City Community Network — 21-site PCMH network across the San Fernando Valley, San Gabriel Valley, and LA County. Lead clinical quality, provider oversight, and Patient Centered Medical Home transformation for a network serving low-income Southern California communities since 1989.
Requirements
- MD/DO Board Certified; executive leadership and FQHC or PCMH directorship experience required
- PCMH model, quality improvement (HEDIS, UDS), and population health management
- Bilingual Spanish preferred — 65% of patients at risk of coverage loss
- Experience leading clinical teams across multi-site FQHC
- Track record in chronic disease management (diabetes, hypertension, asthma) in under-resourced populations
