Primary care physician in PCMH model at Mission City Community Network serving San Fernando Valley low-income patients since 1989. Full preventive care, chronic disease management, Family PACT, STI screening. NHSC eligible.
Requirements
- MD/DO Board Certified in Family Medicine or Internal Medicine
- NHSC Loan Repayment eligible
- Bilingual Spanish required or strongly preferred
- FQHC or community health center experience
- Chronic disease management: diabetes, hypertension, COPD panel management
