Drive NEVHC's quality improvement initiatives — HEDIS measures, UDS reporting, PCMH accreditation, and care gap closure campaigns across all clinic sites.
Requirements
- 2+ years QI or population health experience in a clinical setting
- OCHIN Epic reporting and registry experience preferred
- HEDIS, UDS, or PCMH experience strongly preferred
- Spanish skills helpful for patient outreach
