CHW at Omni Family Health connecting Kern County's farmworker and immigrant communities to primary care, Medi-Cal, and ECM services. Field-based outreach across 26-site service area in oil and agricultural communities.
Requirements
- CHW certification or 2+ years agricultural community outreach
- Spanish fluency required; indigenous language (Mixtec) a plus
- Valid CA driver's license — extensive Kern County field travel required
- Farmworker lived experience highly valued
- Knowledge of Kern County social services and Medi-Cal enrollment
