Requirements
- Active California NP license and DEA
- Family or Adult-Gerontology NP certification
- FQHC or safety-net primary care experience
- Multilingual skills preferred (Spanish, Vietnamese, Tagalog)
- Chronic disease management for under-resourced Bay Area populations
