ECM care manager supporting complex-needs Medi-Cal members in rural Humboldt County — housing instability, substance use, and chronic disease prevalent in this community. Coordinate with local agencies, provide community-based outreach, and connect patients to Community Supports.
Requirements
- RN, LCSW, MFT, or CHW credential; or equivalent experience
- Experience with housing-unstable, substance-using, or medically complex populations
- Driver's license required — field-based outreach across Fortuna, Rio Dell, and Scotia areas
- Cultural humility with North Coast Indigenous and rural communities
- NextGen EHR documentation skills
