About the Role
Provide RN-level care management for St. John's Complex Care Management program, conducting medication reconciliation, transitional care follow-up, and chronic disease management for high-risk Medi-Cal patients across South LA.
Requirements
- RN with California license; care management or community health nursing experience preferred
- Knowledge of chronic disease management, transitions of care, and Medi-Cal CCM billing
- Bilingual Spanish/English preferred; OCHIN Epic care management module experience a plus
- CCM certification (CCM or ACM) a plus; BLS required
