Requirements
- RN, MA, CHW, or equivalent; care coordination or case management experience
- Bilingual Spanish preferred — majority Latino patient population
- Epic care management module experience a plus
- Chronic disease management: diabetes, hypertension panel management
- Health education and motivational interviewing skills
