Requirements
- MD/DO Board Certified in Family Medicine or Internal Medicine
- NHSC Loan Repayment eligible — Alameda County HPSA
- Cultural competency with African American/Black communities; anti-racism and equity-centered practice
- OCHIN Epic proficiency preferred
- Cardiovascular risk reduction, diabetes panel management experience
