Requirements
- CHW certification or equivalent lived experience in community health
- West Oakland community roots strongly preferred — trusted community relationships matter
- Knowledge of Alameda County housing, re-entry, substance use, and social services
- Cultural competency with African American community — anti-racist, trauma-informed practice
- OCHIN Epic documentation; training provided
