About the Role
CHW at AHS connecting East Oakland's most vulnerable residents — unhoused individuals, recently incarcerated, and immigrant families — to Wellness Center primary care, behavioral health, and ECM services.
Requirements
- CHW certification or 2+ years community health outreach experience
- Multilingual skills (Spanish, Cantonese, Amharic, or Tigrinya) highly valued — diverse East Oakland community
- Lived experience in East Oakland or similar urban under-resourced community preferred
- Valid CA driver's license
- Knowledge of Alameda County social services, Medi-Cal, and housing resources
