Requirements
- Active California RN license required
- 2+ years care management or FQHC nursing experience
- Epic EHR proficiency strongly preferred
- Spanish or Cantonese language skills a significant plus
- Experience with unhoused patients, complex chronic disease, or behavioral health comorbidities
