CHW at The WeCare Group serving Trinity County's frontier Mad River corridor with 3 sites. Community health outreach for isolated rural residents, connecting elders, ranching families, and tribal members to health services across vast geographic distances.
Requirements
- Experience in rural or frontier community health outreach
- Valid California driver's license required — extensive local travel
- CA CHW certification preferred
- Ability to work independently in remote settings
- Knowledge of Trinity County resources and rural health challenges
