Palau is not a U.S. state or territory — it is a sovereign nation in a Compact of Free Association (COFA) with the United States, so there is NO on-island U.S. Medicaid or Rural Health Transformation Program eligibility; Palau's own health system runs on renewed-Compact federal assistance (the 2024 amended Compact / Compact Review Agreement provides ~$889M over 20 years through FY2043, including dedicated health-sector grants). For U.S. FQHCs, the Palau angle is the Palauan diaspora: COFA migrants lawfully present in U.S. states — concentrated in Hawaii, Guam, Oregon, and Washington — are one of only three immigrant groups whose full Medicaid/CHIP eligibility was PRESERVED under H.R.1 (Public Law 119-21) §71109. Beginning October 1, 2026, federal matching funds for full Medicaid/CHIP are limited to lawful permanent residents, Cuban/Haitian entrants, and COFA migrants, and CMS guidance (SHO #26-001, April 8, 2026) directs states to file the conforming State Plan Amendments by December 31, 2026. FQHCs serving Palauan communities should treat their COFA patients as a protected, continuously-eligible population even as broader noncitizen Medicaid coverage narrows.

FQHCs bill for an embedded behavioral-health care manager plus a consulting psychiatrist supporting the primary-care team — integrated mental-health care is billable. (CMS retires the G0512 bundle in CY2026; FQHCs move to component codes.)
Transitional Care Management (TCM)
