ECM Care Coordinator role at Alliance Medical Center. Manage high-risk Medi-Cal patients — farmworker families, seasonal agricultural workers — through CalAIM Enhanced Care Management. OCHIN Epic documentation, 20-patient panels, community-based care.
Requirements
BA/BS in social work, public health, or related field; or 3+ years FQHC care coordination
Spanish fluency required
ECM or CCM program experience strongly preferred
OCHIN Epic proficiency preferred
Valid CA driver's license for community-based care visits
