How the program works

• • $50B over five years (FY2026–2030), created by H.R. 1 and run by CMS; year-1 funds are generally spendable through September 30, 2027.
• • States apply and sub-grant the money — each state designs its own tracks, deadlines, and eligibility, which is why FQHC access varies so much below.
• • States only: DC and the territories (Puerto Rico, Guam, USVI, CNMI, American Samoa) are excluded from the program entirely.
• • RHTP money is new, and it does not backfill Medicaid cuts. Several states' own health officials have said so on the record (see the state feeds below).
