About the Role
Lead a team of ECM care coordinators and case managers supporting AltaMed's high-utilizer Medi-Cal patients, overseeing complex care planning, multidisciplinary coordination, and CalAIM program compliance.
Requirements
California RN license required; BSN preferred; case management certification a plus
3+ years complex care management or ECM experience in FQHC or managed care
Bilingual Spanish/English required for direct patient and team communication
CalAIM ECM lead entity experience; familiarity with enhanced care protocols
