About the Role
Coordinate home-based services and community supports for AltaMed PACE members, conducting home assessments, arranging attendant care, and supporting the interdisciplinary team in comprehensive care planning.
Requirements
Associate degree or equivalent in health or social services; BSW preferred
Bilingual Spanish/English required — predominantly Spanish-speaking PACE members
Experience with frail elderly populations; home health or PACE knowledge a plus
Valid CA driver's license required for home visits
