About the Role
Provide primary and specialty geriatric care to AltaMed's PACE and senior patient populations in Orange County, focusing on chronic disease management, functional assessment, and geriatric syndromes.
Requirements
MSN-NP with California NP license and DEA registration; Geriatrics certification preferred
2+ years geriatric or PACE NP experience in FQHC or senior care setting
NHSC loan repayment eligible — AltaMed qualifies as HPSA site
Familiarity with FQHC PPS billing and Medicare dual eligible patients
